A Smiley Face New Jersey (“A Smiley Face,” “we,” “us”) provides ABA therapy and related services for children on the autism spectrum and their families. Our center is at 200 Middlesex Turnpike, Ste. 307, Iselin, NJ 08830. This policy explains how we handle information collected through our website, asfnj.com.
Information we collect
Information you give us. When you use our contact form, our new patient intake application or our job application, we collect what you enter, such as your name, email address, phone number and message. The intake and job applications may also ask for information about your child, insurance or work history, and for documents you choose to upload.
Information collected automatically. Like most websites, our servers and service providers record technical information such as your IP address, browser type, the pages you visit and the date and time of your visit. We also use cookies and similar technologies, described below.

Health information
Please don’t send detailed medical information through our general contact form. Health information you provide as a client of A Smiley Face is protected under federal and state privacy laws, including HIPAA, and is handled according to our client privacy practices, which our office can provide on request.
Forms and third-party services
We use trusted service providers to run parts of our website, and they process information on our behalf:
- Jotform hosts our new patient intake application and our job application.
- Google Analytics helps us understand how visitors use our site, using cookies and anonymous usage data.
- UserWay provides our accessibility tools (such as larger text and higher contrast).
- Google Fonts supplies the typefaces our pages use, and Google Maps opens when you choose “Get directions.”
- GoDaddy hosts our website, and Cloudflare helps deliver and protect it.
- Instagram: we link to our Instagram profile; Instagram’s own policy applies when you visit it.
These services have their own privacy policies, which govern the information they collect.
Cookies
Cookies are small files stored by your browser. We and the services above use them to keep the site working, remember preferences such as accessibility settings, and understand how visitors use the site. We do not sell your personal information. We share it only with service providers who help us operate our website and services, when you ask us to, or when required by law or to protect the rights and safety of our clients, staff and others.

We keep information only as long as needed for the purposes described here, or as required by law. Client records are kept according to the record-keeping rules that apply to healthcare providers.

We use reasonable safeguards to protect information, including encrypted (HTTPS) connections. No website or online transmission is completely secure, so please contact us by phone if you prefer not to share information online.
Children’s privacy
Our website is intended for parents, caregivers and job applicants. We do not knowingly collect personal information online directly from children under 13. Information about a child is provided by a parent or guardian as part of an application for services.
Your choices
You can ask us to access, correct or delete information you submitted through our website by calling (732) 510-7779 or using our contact form. We may need to keep certain information when the law requires it.
